Abnormal adherence of the placenta to the myometrium is established in very early pregnancy and can be subdivided into placenta accreta (where chorionic villi directly implant on to the myometrium), placenta increta (where chorionic villi invade into the myometrium), and placenta percreta (where chorionic villi invade through the myometrium and may involve surrounding structures) Placenta percreta is a term given to the most severe but least common form of the spectrum of abnormal placental villous adherence, where there is a transmural extension of placental tissue across the myometrium with a serosal breach.It carries severe maternal as well as fetal risks Placenta accreta is different from Placenta increta (invading into the myometrium) and placenta percreta. Microscopically, the placental villi interdigitate directly with the uterine myometrium, without an intervening decidual plate. This is placenta accreta, which will result in lack of normal separation at delivery, leading to marked hemorrhage necessitating emergency hysterectomy
